A MD/Physician query is a powerful communication tool used to clarify documentation in the health record to achieve accurate code assignment.
Our MD/Physician Query Tool Set provides a structured, customizable method for querying the physician when the diagnosis provided is incomplete, inconsistent, or conflicts with other documentation of diagnoses in the medical records, also known as Clinical Documentation Improvement (CDI)*. Queries include most frequently questioned diagnoses and address PDPM case mix diagnoses for nursing and non-therapy ancillary (NTA).
