A MD/Physician query is a powerful communication tool used to clarify documentation in the health record to achieve accurate code assignment.
Our MD/Physician Query Tool Set provides a structured, customizable method for querying the physician when the diagnosis provided is incomplete, inconsistent, or conflicts with other documentation of diagnoses in the medical records, also known as Clinical Documentation Improvement (CDI)*. Queries include most frequently questioned diagnoses and address PDPM case mix diagnoses for nursing and non-therapy ancillary (NTA).
Examples of MD/physician queries in the set include:
- A-Fib & Flutter
- Embolism and Thrombosis
- Morbid Obesity
- Myelodysplastic Syndromes
- Psoriatic Arthropathy & Systemic Sclerosis, and more!
A full listing of the queries can be found in the Table of Contents here.
*CDI is the process of enhancing medical data collection to be more specific as to the condition or diagnosis to support the quality of care provided, maximize and support the level of claims reimbursement revenue, and improve care quality.
